The deadline for the receipt of completed registration forms with signed waiver and payment is May 1, 2008. Players registered after this date may be placed on a waiting list.

Please make your cheque payable to to the ARC SPORTS PROGRAM - Please mail cheque and waiver form to the Registrar, Annette Baseball Association, P. O. Box 84599, 2336 Bloor St. West, Toronto, ON, M6S 4Z7.

As this form will be the only copy issued to you, you may wish to keep a copy for your records.
